Jednocześnie oświadczam, że przekazuje moje dane osobowe całkowicie dobrowolnie. Oświadczam ponadto, że zostałem/łam poinformowany/a o przysługującym mi prawie do cofnięcia zgody lub zgłoszenia sprzeciwu wobec przetwarzania danych, żądania do nich dostępu, sprostowania, usunięcia, ograniczenia przetwarzania oraz ich przeniesienia, w każdym czasie.
